Guardian authorization and participation
Required acknowledgment
I confirm that I am the child's parent or legal guardian and have authority to submit this application. The information I provide is accurate to the best of my knowledge, and I will notify AAI of changes to contact details, authorized pickup persons, health information or support needs. I request participation in the program identified in this application, subject to staff approval and any stated eligibility or audition requirements. Submission does not guarantee a place. Any field trip, transportation or activity outside the described program requires separate arrangements and any additional permission AAI requests. By typing my name and submitting, I intend to record my agreement to the required documents and only the optional permissions I select.
Safety and emergency assistance
Required acknowledgment
I understand that theater, dance and movement involve physical activity. I will share relevant allergies, medical concerns and support needs with AAI and discuss any needed accommodations before participation. I authorize staff to contact me and the listed emergency contacts and, when necessary, call emergency services and request emergency evaluation or assistance for my child. This does not authorize routine treatment, medication administration or non-emergency procedures; those require separate arrangements. This acknowledgment does not release AAI or anyone else from liability or waive legal rights.
Attendance, conduct and pickup
Required acknowledgment
I will review the program schedule with my child, communicate absences or late arrivals to AAI, and arrange timely pickup by a person I have identified as authorized. I will discuss schedule or transportation difficulties with staff. My child and I will follow reasonable staff safety instructions and treat other participants, staff and facilities respectfully. I will discuss concerns with AAI so staff can review appropriate next steps. This acknowledgment does not establish an automatic suspension, removal or refund policy.
Enrollment records and service messages
Required acknowledgment
I understand that the website stores account, household, emergency contact, child, application and consent records for AAI's enrollment administration and staff review. The website uses hosting and email service providers to operate these functions. I authorize AAI to use the information I submit to review the application, communicate about enrollment and program matters, and support my child's participation. This is not permission for unrelated marketing or public disclosure of my child's personal information. I can contact info@asanteartinstitute.org to request corrections or discuss access, retention or deletion of records.
Application review, fees and membership
Required acknowledgment
I understand that applications require staff approval. The Academy currently displays a program fee of $0.00 and $0.00 due today. Asante Children's Theater membership is a separate item marked Fees Apply; it is not automatically free. A new child's membership term is four years. A child with an existing active membership keeps that membership until it expires rather than receiving a new term with each application. Billing rules are still being determined. This submission does not collect payment, authorize recurring charges, or accept an undisclosed fee. Any applicable charges and payment arrangements must be communicated separately before payment is requested.
Optional photo, video and audio permission
Optional permission — leaving it unchecked does not grant permission.
If I select this optional permission, I authorize AAI to use photographs, video or audio of my child's program participation in AAI's program communications, website, social media, printed materials and fundraising communications, without payment. This permission does not authorize publication of my child's full name, personal contact details, school, health or household information. Leaving this box unchecked means I do not grant this permission and does not prevent submission or eligibility. I may request withdrawal for future use by contacting info@asanteartinstitute.org; material already printed or shared by others may not be fully recoverable.
